I've started to take my deep seated dependance on right rotation seriously.
I've been playing since the NES version came out. While I played some arcade, NES and gameboy, the version I played the most was on windows 3.1 (gosh, I'm old) and used a very similar keyboard setup to the TF default. I don't recall if CCW rotation ("left") was even possible.
I started the change by using the number pad (up/8= HD, down/2=SD) and A,C, and Shift over on BB with driller mode. I pretty quickly shifted to 40 lines. TF sprint was much more difficult because the only way I can customize my inputs from default is to use IE 7 (Firefox and Chrome won't work the letter inputs at all.)*
I've backed off the numberpad and gone back to the arrow keys for now, but the real issue I'm having is thinking about rotations before I do them. My 40 lines times are horrendous now. I'm taking 6 to 8 times longer than before and I'm effectively relearning stacking efficiency.
I was rotating CCW for twists and spins already, but things like rotating I, S and Z in the direction I want them to go faster requires active thought.**
Finally, I reverted back to shift for hold, to avoid and excessive confusion, but given that my left hand is holding in a different position and accessing different keys, there is already some muscle confusion.
It's day 2. I'd thought it might take a month to get back to where I was. It's so frustrating and exhausting that it may very likely take longer. It's much more like doing physical therapy to relearn basic motor functions than I could have possibly imagined.

*[I've had this problem for quite a while with TF and the letter inputs where it primarily wouldn't let me use letters (in this case the letter "V" for items) during the Arena Challenge (or I could do it once per round and then it wouldn't acknowledge any inputs until the next round). Because the only version where this didn't happen is my old IE version and it lagged so bad, I just stopped doing any ACs with items.]
**(As an aside, the I, S, and Z pieces should just have one state and that all the kicks and what not WRT to whether you input them as CW/CCW is total BS. The way S and Z specifically won't kick up if you rotate the wrong way is super counter intuitive)
